Recognition Certificates

Need help with Arabic to produce certificates?

If I am going to award a certificate to a male participant, should I write this way: ومُنح هذه الشهادة تقديراً لحسن جهوده Or: ومُنحت هذه الشهادة تقديراً لحسن جهوده مُنح or مُنحت Thanks!

1. ومُنح هذه الشهادة تقديراً لحسن جهوده ومُنحت is for female
2. Both are correct. The first says "he was awarded this certificate". The second says,"this certificate was awarded". In the first, "he" is subject. In the second, "the certificate" is the subject. If the certificate is prestigious I would go with the second choice, i.e." this certificate is...".
3. bander is not accurate. The Korean fellow is right.
